Brothers John and Matt have inherited an island cabin from their recently deceased uncle. Along with Matt's girlfriend, Nicki, and other mutual friends, the siblings travel to the cabin for a relaxing weekend getaway. But, not long after arriving, the group is besieged by ravenous dogs. They watch in horror as another vacationer, Luke, is eaten alive. Soon, they discover a training facility where the dogs have been bred to kill.
In 1969, while studying at the University of Maine, artist Alan Parker becomes obsessed with death. Believing he is losing his girlfriend, he tries to commit suicide on his birthday but his friends manage to stop him. He receives news that his mother has had a stroke and decides to hitchhike to visit her at the hospital.

As more murders occur, police investigator Buckley Clarke reluctantly joins forces with Sam. The brass are convinced this is a serial killer choosing victims at random, but Sam and Buckley discover a pattern involving health personnel who work in neonatal intensive care. Doctors aren't supposed to play god, but someone's decision years before has driven a killer over the brink. Can they find him?
